the tool do more,... it REMOVE the hack too,..
at the end from the game
1) it remove the modified "max zoom out" value to the original value (2250 float)
2) it zoom in and zoom out with the mousewheel (this is important for the "current zoom out" address)
3) it click the continue button

ofc it need about 3 seconds to remove the zoom hack...
This is the reason why i freeze your mouse and keyboard at the end of the game with the (Automatically Remove Function), for 3 seconds,...

I found out that changing the Windows theme from Aero to Basic reduces the fps spikes greatly and makes it much smoother, but still has some small fps spikes and screen tearing.

How high is your "max zoom out" value?
First the zoom hack is not the problem because the zoom hack only modified your "max zoom out" value, it do not more,...
disable the function (Automatically remove), if you still have problems change the priority of your lol.exe process to "high".
If you still have the problem say thanks to LOL because they dont support your graphic card to 100%,...
