[HELP]Adding Skill to Item

I want to add some skills to Items.
I tried to add but failed :/

I tried to do as follows;
_RefAbilityItemOptLevel
1 91 2164 0

RefSkillByItemOptLevel
91 45000

same as media.pk2

but doesn't work.

help me please.
