Adding new skill in the skills window

Hello epvpers So last few days I'm trying to figure out how to add new custom skill in the skills window (S) but I keep failing and failling.
This is what I've got at the moment:


As you can see there is no skill icon, also when I place cursor over the skill icon it doesn't show anything. And pressing 'Add' leads to the client crash.
Maybe anyone can help with it? Thanks!

One guy from rz gave me an advice. It was duplicated GroupID.
