great question. no clue.
probably an error with the directx transforms, try updating to the latest version of direct x and the hack.

also run in compatibility mode for win 7 32bit and update drivers.
dont forget to run as administrator.

Not yet I dont think...I've been trying to be careful when using it. I've mostly just been using the rocket-jump ability just to do vistas and jumping puzzles. Hopefully if I do get spotted they'll be less likely to report since I'm not hurting anyone.

you are not hurting anyone directly but indirectly 'cause you use unallowed shortcut-methods to get achievements faster than others can... and that makes them report you 'cause they think its not fair if someone can be faster than the others

yeah but if someone reports anyone in this game they are just making it harder for themselves, if nobody is hurting you, leave it be, and go about your adventures .. i think most people are like that, unless... they are young, they might have a less mature view of it.
