[HELP] what am i doing wrong with register?

I'm having problems getting a register page up...
Can someone tell me whats wrong with my registering files? ill attach them here.

I put in the correct password and everything. i type in the correct site link and everything(5.78.241.48 for me). so can someone look at these and tell me what im doing wrong?
