Your discord may have been token logged via your memory which can't be detected unless you know the signature. Discord PTB new update now allows you to see active sessions (image linked).

If any of the sessions come up as "unknown" or from a location you do not recognise then you have most likely fallen victim to a token log by a dev which you can now remove and resolve.

Now we just need an update to block server members from DMing other members if they are not their friend list. As an option to avoid other server bot spam if you know what I mean. (I am not talking about the option available in your profile)

I believe you can turn it off via privacy settings. Privacy > Allow direct messages from server members. Which will only allow messages from friends and not randoms from other servers.

Discord recently added a new tab in their settings called “Devices” or something like that, it shows you everywhere you’re logged in at. If you’re worried about being ratted, check that tab and make sure you recognize all the devices.
