From the top of my head.
But, biggest reason (IMO) is, you're forcing players ingame to do stuff they might not want (or doesn't fit their build/char) because of "muh activity based". It creates environment where 10% of addicts will jump through all of your hoops and be "pros". Over time, the rest of 90% aka "plebs" will come into contact with "pros" and think to themselves:

Eventually plebs leave, then there's no one for "pros" to shit on, so they leave too and server dies.
For 12 years entire dev "community" is basically catering to the addicts and disregarding plebs (majority of players). On top of that, player base is shrinking as people get older and no one is willing to put their limited free time into the game to make +7 set, only to be a punching bag for +12 "pros". People are moving on with their life.
Also, most of the new "content" that I saw being added ends up being nothing more than a chore.

Entire game is designed as a gigantic time sink, where the top of the pyramid is so far ahead, that the bottom of the pyramid can't deal with it, so entire thing collapses.
Sometimes I wish I could code, I've had a couple of ideas for this game over the years...
