Did they do anything ever since? New Caps? New Areas? ( I have seen alexandria)
Is it worth starting again?

Yep, plenty of new updates starts with the Egy-weapons, new trade routs, job cave
(Consignment Trading) kinda hard to explain that

Constantinople Fortress .. plenty of new stuff.

But, nah it doesn't worth to start it again in my opinion unless if you're with a group of players/friends
like me and Porta-kun (@)
you can pick random private servers and check what you missed there just if you're curious.

are you thinking of starting in isro or sro R?Then please don't i recently left my sro r (Sold all my chars and gear) its like dead with maybe 1 or 2 guild and FW is a joke with 2 parties on both sides. thats my opinion thou.Joymax also added Alchemy for 12D-15D that you will need to spend silk to buy and its just a money pit for that.

Pservers is where its at. Yea its not a forever server but you will get the most fun for it . Like 100x Fun.
