make sure you type the code with '\' not '/'
example :
item\etc\avatar.ddj
and also make sure that u add .ddj at the end of the code.
if those 2 solutions didn't help, so you have to make sure that you have added the .ddj icon

NPC image from ( refpackageitem ). check the ddj in there

As far as I remember "icon64" is for the npc/IM path and "icon" is the image when the item goes to your inventory.

nope, icon is for NPC
when it goes to your inventory it's itemdataxx
icon64 is for Item Mall
