Just the act of modifying a program's executable file will drive most anti malware setups crazy. When you patch an exe file, you make the chcksum generated by the program's linker invalid. Antivirus programs detect that and flag it positive.
Some game devs will run the exe through an exe compressor to get away from that. In most cases that works, but there are a few some anti malware scanners that will treat the new decompression functions as a trojan injector.
At the end of the day look at what it is that is being flagged and ask yourself
"Would it have been necessary to patch that program?" and if you answer
"yes" then you can pretty much assume the exe is safe to use and being flagged simply for that modification.
