To help you I will only say this, the rest you must figure out yourself:

Game Server opt: IP Needs to be 'External IP' ()
All DB Options in Game / Auth Opts: 127.0.0.1

You Need All of the following Forwarded in BOTH Windows Firewall + (IF YOU HAVE A ROUTER) Router:

1433
3516
4500
4502
4514
4518

These must be forwarded to the correct LOCAL Address (Can be found via IPCONFIG [CTR + R] type [CMD] press [ENTER] )
