Well I wouldn't go as far as slamming Mods with a "use at your own risk".. more like " don't use it if you don't know how or what its about. " -_-;....
like I said.I wouldn't say use at your own risk, More like Scan before you use it, and how it works before you attempt to implement it into your game. If you are going to use a mod, and its from a posted forum ( much like this one) normally you will want to see who it is that posted the mod ( rep can't always be trusted but helps a lot), and if its not theirs check the source forum or the origin of the mod itself. As long as you can see the mod has been shown tested by other before you and works and its been approved to work and its clean. then you should be good.
Most mods (unless super obvious ) are not very detectable mostly client side, as long as you follow procedure you can normally run a mod without any harm. It's all about how you go about it.
Now the only other thing that could be unsafe is how you handle yourself in games that you have the mod on, if you go around stating

Mods require a level of maturity when using them, knowing what is a good one and what is a bad one. Educate yourself, become a member of a UG community that has respectable Mods that have not shown to have irreversible consequences.
As far as your questions about Nexon seeing things with their blind eyes... X_X umm its really rarely proven that they look for these things. normally if its been leaked that its being used they can normally attempt to patch it, but server logs don't normally log client modified data such as that, that normally would require some hefty network configures and to have something like that constantly pinging your connection looking for modifications like that can create degradation on your connection. This is why Hack shield was implemented, So that it can detect modifications to your folders. and its all on your side not Nexons. i might be wrong but that's my best observation.
