Looking for a picture of the back of riot ward code

Hello guys Im looking for a picture of the back of a riot ward card. I have a code that expired, and the only way riot will let me redeem it is If i have a picture of the back of the riot ward card and my summoner name written on a piece of paper next to it.

If someone could send me a picture I would really appreciate it!
