Because I don't have access to a database, I can't see the column headers to ensure you have them in the right fields, however the cq_npc and cq_task entries have the correct data. However the cq_task is missing an entry. You don't need to have a cq_task entry for every action you have, just a task based one. Actions are called from tasks and this is when the "action limit starts" so you can start a new action line by calling a task. One such task calling action is the type 102 which has the following parameter setup: "Action~Display~Text~Without~Spaces TASKID"
The only issue with your cq_action is the following:

Also the part that idle is talking about "npcendaction" may not work for you, in my database for example I created 100 actions with the type 104 and the parameter for faces between 0 and 99, and each one then pointed to type 120. this allows for saving entries in your database, decreasing load time
