When I had this done, they wanted:
1. A copy of a government-issued ID (if you wish you can block out the photo)
2. User ID / server for the account
3. The very first username for the account
4. The last 2 to 4 passwords for the account
5. Proof of payment transactions (minimum of 4). Can be transaction numbers, payment receipts, mobile number in the case of purchases via text message)

Got my accounts back over 2 times already.
But the info about id, bank transfers etc etc is true.
Support wil give accounts back.
But i read allot of people didnt get it back.
And i cannot understand why they wouldnt do this...
This is seriously sad if they would not give hacked accounts back...
Or restore them to the day it got hacked...
