Look at the table structure in TQ binaries or w/e.
Each item has a fixed percentage associated with it. They run their RNG and use that to determine which reward is handed out.
Not sure what you expect us to say lol. It's no different really from DBs dropping from monsters except there are more rare items all with their own percentage chances to be dropped/won.
