Don't quite know what you are saying but I will try my best.

A server cannot truly be 24/7 as you need to send out patches and updates to the game clients. For this you will need an autopatcher or something of that sort.
To update the source you will need to add stuff like NPCs and other stuff and then close the server and rebuild the server. Once you have done this the server can then be reopened for people to play.
IF you are only making new NPCs that have the "stock NPC" Names then no updates will need to be sent to the clients.
This is the same for most things unless you are making client side edits. e.g the wat stuff looks like the interface, Scrolls, Armor and stuff like that.
Also what do you mean "if I have the source open"?
Do you mean it will only work in debugging mode?
If this is what you mean open up OldCODB\bin\debug and from there run the application Called NewestCOServer or whatever the source is called.
