They do not necessarily need his personal information. For example, to scare him, they could send an email to a business email address such as victor@hypervision.com (not his real email, just an example) without necessarily knowing the identity of the individual behind that address.
BUT, his decision to shut down strongly suggests that they did have identifying information, and it's reasonable to infer that they sent a demand letter, likely accompanied by a notice of intent to pursue legal action, identifying him personally and demonstrating that they had sufficient information to establish his identity.

I personally can't say for certain. My firmware is extreme overkill for COD, so I never have to worry about bans. All of my friends use, at minimum, basic paid firmware, so I can't speak to whether they've implemented a library ban against free firmware.
The only real way to test it would be to spoof, use a throwaway account, run free firmware for a while, and see what happens.
Acti probably found the person behind hypervision and served them papers like Elocarry. Those court orders are known for being gag orders a lot of the time as well.
Yeah i’ve been hearing this for months now, still nothing, anyhow most decent emulated fw nowadays support the iommu thing bro, i know the one i have does, they gonna need more than that to get us, look at bf6, showed some hope but nothing happened in the end.
For COD in the current state, DMA is by far the safest bet by miles.
[CENTER]Dma is the safest but your average cheater still will go get an external and play normally. Also the most expensive and DMA can be detected as well if its not updated, but thats all depending on the person where they go to purchase lol
