The spoofer will have to utilize seeding to be able to generate the same HWID each time you spoof. The seed is used to generate the hwid, in short: if the seed changes so will the HWID.
Not all spoofers do this but they really should as its straight up reckless not to. It'll also look extremely suspicious to any third party investigating your account if you dont and keep changing hwid each time you enter the game.
Cheat providers also appreciate seeding as they'll be able to hwid protect their softwares using common techniques rather than having to result to uncommon special tricks to work with the spoofer.
As for your last question, no generally not, most people utilize a spoofer to protect their real hwid, but this sort of falls in under what i wrote earlier. If an update comes and they start pulling new different identifiers off your machine with different techniques that the spoofer isnt aware of and you start logging in to both accounts without being aware, then yea in a worst case scenario it may happen. However, I am yet to see this from COD, but i have seen it from EAC. I'm only being realistic here, if anyone tells you otherwise they simply don't know what they're talking about.
