Then it's pure chance. Out of 2+ years of playing I've gotten ONE super item and it was on the first rock monster I killed randomly. If anything is given a flag to have better chances, it'd be the spawn as a whole. Perhaps the first monster killed gives a better chance at dropping something. Other than that, everything else would be random. If you consider it, the odds are very slim, but with ALL the people in game at once hunting stuff, you know SOMEONE is getting SOMETHING at just about any given time. Say 1k people are hunting. You're sure a +1 is dropping every few seconds to few minutes. That's ONLY because so many monsters are being killed. When YOU get a rare/+1/whatever, you just happened to be one of the lucky people currently killing to have the probability say DROP HIM AN ITEN OLOLOL. Also, people say "why don't supers get +1s often?" You're combining probabilities which result in EVEN LOWER chances. If you take 10% on super and 10% on +1, to get a +1 super would be 1%. If it's a 10% chance for 1 socket and 1% chance on 2 socket, then a 1s +1 super is 0.1% and 2s +1 super is 0.01% chance. You can see how it just keeps dropping (real odds are moreso around 0.1% chance each and not 10% meaning you're looking at odds added up around 0.0001% for a super rare in one drop).
