Both. You can get banned automatically even if nobody reports you if the system somehow thinks you've been cheating. But if you were reported, an EA employee will check your account manually and then decide whether to issue a ban or not.

3 warning = ban I guess

If you don't want get ban, Use up to 2-3 million coins and build your squad. If you get 2 warning stop use trainer.
