@OP Support will ask you for (full list, you might not need all of these info though):
• Your full name, which should match the one that might be registered on the account
• Your username
• User ID
• Your email
• Server
• Last connection to the account (by you)
• Account's first email
• Account's first username
• Account's creation date (approx.)
• Payment proofs and the email which you've received them on
• In case they were any SMS payments, the phone number you purchased with
• In case they were any credit card ayments, the name written on the card
• Description of the issue and how it happened
As well as :
• A scan/copy of your ID card / a valid identity proof
• A police report
• Payments proof copy
