Actually swansong posted the image for me (my images don't show yet, guess is 'cause my acc is new).
None crystal failed, but I always use great fortune manual.
I opened one pack of 3 stones, then 2, then 1. After that only box. The average is 1-2 per box. The max I opened was 4 crystals.
