DDJ Is for pictures . not icons only, Teleport screens etc. are used as ddj.
BSR as I know , It's 3d format inside data.pk2
BSM same as BSR .
And , you cant use 1d format as a 3d , nor appositive.

DDJ: Icons / Ingame Images (teleports, loadings, etc) / Textures
BSR: Item's/npc/monster appearance
BMS: Item's shape, this can be opened converting the selected .bms file using perrytje's Model converter ([Only registered and activated users can see links. Click Here To Register...] and opening it with 3D programs like Blender.

Correct me if I'm wrong please.

They are mostly at Data.pk2. \prim\mesh\item\
Example: Data\prim\mesh\item\china\weapon\blade_01.bms
Explore inside the pk2 files, you can find everything in there.

I don't know how to add a new one but for example, you can edit an existing one.
1- Export a .bms file that you want to edit
2- Convert it to .obj with [Only registered and activated users can see links. Click Here To Register...].
3- Open it with Blender.
4- Edit it.
5- Convert it back to .bms.
6- Export it with the same name it had to the same place where it was.

Wear it in-game, it should look edited.
