Late Game Failing

Yo,
i got a Problem always if im playing i get fed in early, because im doing really great plays. Than its going back from minute to minute i dont know what i do wrong. I try to help other lanes or push towers drags etc. sometimes work sometimes not. And than i die to fast or something else, and this happens over and over and than from 3/0/0 to 3/6/6. I try to stay behind the fight so i can go in, but i always die directly. I fed myself everygame mostly in early but i always fall off in late you guys got any videos for Lategame tipps or some tipps in general would be nice.

Champs i play: Riven,Pantheon,Lee,Vayne,Cait,Syndra,Ahri,Rammus ect.
Division: Gold1 dropped from Plat5

You should Snowball harder. Seems like you help your mates to hard. Do the Dragon is the best you can do as a AdC/Jungler. Gank Lanes is only useful if you can really do something, you can easy miss 2 waves of Minions while helping a lane. And work on ur Positioning! "I try to stay behind the fight so I can go in, but I always die directly" don't sounds like a Gold Player!

And don't forget, Qucksilver Sash is very, very useful against CC.
