Here are the instructions roughly translated into German, hope it helps:

1 Schließen Sie sich einer BF3-Server
2 Warten Sie, bis Sie im Spiel sind
3 ALT + TAB und offene Extreme Injector V3.3 und gehen Sie zu Einstellungen - (Achten Sie darauf, nur Extreme Injector V3.3)
4 Verwenden Sie die folgenden Einstellungen:
Injection-Methode: Manuelle Karte
Überprüfen Sie beides: Löschen PE, Erase Module
Scrambling-Voreinstellung: Extreme
5 Spritzen Sie das Hack in bf3.exe
6 Wenn Sie im Spiel drücken Sie F8 sind
7 Genießen Sie!

Have you ever used other hacks? If you used any other hacks in the last few weeks then that could be why you are now banned because it seems like a new ban wave was just put into effect. PB does silent logging so they could have logged your account using other hacks weeks ago and are just now banning you for it.

This still works for me just fine without issues.
