Equipment behavior set to "Toggle"
Need knife as lethal, because it makes melee animation longer. Tactical must be throwable (smoke, stun, flash)
Melee+Tactical+Fire ...Then switch weapon to put tactical away. And you're back to underbarrel launcher. Press macro button again for next round. (Mine is hold to repeat)
This will eat ammo from your magazine instead of your underbarrel ammo. I use a weapon with a 60 round mag, and I don't have to worry about it. If it stops firing, switch back to "standard fire mode" (regular bullets) and reload. Rinse and repeat.

I'm still holding down Melee and Tactical, when I've already pressed and released Fire. Melee, Fire, Tactical is also the downpress order, I've got. I just recorded keypresses once I could do it consistently. Then altered delays.
The above suggestion regarding a private multiplayer match is exactly what I did.
Also, B-man has a video that's a month old describing how to do this. That dude is extremely cringe imo, but his info is good.

Delay 100ms (Delay is not necessary. Set according to your preference. It was way too fast for me. Burns through rounds like an MX Guardian.)
My previous macro would not work with certain weapons. I still can't use the Cronen Squall with this, but it works consistently with more weapons than my previous setup. I also wanted to ensure that I never actually use an "underbarrel round", because I don't want to be stuck in a reload animation when I'm ready to spam a wall with drills or a doorway with nades.
