I believe its not per win, it is actually from rank to rank. Correct me if Im wrong, but I believe this is how this is done with most providers.

Which may I add, I think is a respectable amount considering the skill level of that rank and the games generally take around 45 minutes each. Bare in mind, you are also playing 4-5 games per rank anyhow so paying 9-12 dollars per game isn't too bad.
