Accessories wont destroyed even failed however if it failed at +5 and above there is chance of reducing the enhancement of 1 or break it (it also reduce the enhancement by 1) but you need to use Repair Hammer to fix it.

Enhancing of Armor and Weapon:
• You can enhance your weapon and armor using old mechanics (Tamir). Its maximum enhancement is +12.
