Well the code itself means D15 Protector advanced elixir (B) You sure everything is correct about it in your DB ? It says it cant find Item itself , so it might no be added . Pretty much common sense, not much of an expert there tho .
Maybe there is a more serious issue , but if you check out this first , and you can find the problem , you can search for it and fix those.
