Remove/add gems in a item

Does it work to Remove/add gems in a socket on an item while the item is equppied? and if it does? how do i do it?

I dont beleive there is anyway to do this. Although i have tried to use the multi tool hack to try and socket a gem into an item < of which it dint work. Maybe patched over. But neways i dont think u can socket items while ur using it.
