i have item from my new NPC so i cant to rebirtrh use it when someone uses its change name how to work with it?

I think you used it wrong. Use the item as a normal Item. Not usable or something. Then you have to make a SQL Script. Write a script for item removal and level downgrade, that means you have to say if ItemKey of this Item is registered in the DB, the character that carrys this item should be set back to level 1 and his exp changed to 0. I can't tell you how exactly this script has to look like, but that would be a possible way.
