maybe there's some missing extensions. try installing everything related to odbc/sql @ iis

That is not extension/IIS/web server related, it simply can't connect to your database.
Make sure you have forwarded the port,enabled remote connection, and so on.
If the website in the same dedicated/vps your server is then use SQL Instance
XXX\SQLEXPRESS or w/e
and check if it's the right port by simply going to sql configuration manager. and check your TCP port there.
