There's 2 possibilities.
#1: The item is not setting its durability properly when read from database.
#2: The iteminfo packet is not being populated/structured properly for your client version.
Either way it requires you to do the most basic of debugging to solve your issue. Breakpoint those two areas and you'll quickly see what's going wrong and should be an easy fix.
