Problem With Forum-Page

I make Forum by me ,
And , if i want add 'Login Panel'
And :
Id must contain with rappelz-account-id
Password must contain rappelz-account-password

like at 6epic private server we have [ admin/admin ]
so how to make .php script for that ?
