how i make reg page connect my ip like this ...

please i want to know how can i make a simple page reg for my private server to let people reg on my game and make site like this

i know this is a HTML codes but i don't know how to make ,, so please and one can help me to make it :)

Download PHP, Apache.
Instal both.
In Apache/htdocs directory create register.html
Within that Register.html file create the registration form.
Pass the submitted form to e.g. process.php.
Establish a connection with the MSSQL server instance in the process.php.
Catch the form - sanitize the input data.
Create a query: INSERT INTO /users_table/ (username, password) VALUES ('$username', '$password').

And you are done. ;)
