Server status

I need Server status for 5375 source.
example:

Server is: Online
Total players online

Can anyone give me it?

This is already released a few times, i suggest you to use the "SEARCH" button..

I was searching almost 2 hours and i didn't found anything.
I found one and i put it but when someone login it show 2 players online and it's 1 player online.
If there is 2 players online there is 4 if there is 3 on status is 6.

It's not for my source.
In my accounts database isn't online column.

I modified that exact same release for MY source in less than 10 seconds by just changing "characters" to "entities" and changing "Online" to "LastCheck"
