+, pct and bless are created when you pick the item up, or atleast they cannot be displayed via the client until you pick it up; Sorry.

How many times do I have to tell you. + items and Blessed items CANNOT BE SEEN ON THE GROUND WITH ANY HACK.

They are decided when you pick them up off the ground.
