The form should be like this:

Upload on imageshack then you can see a bar at the left, copy and paste the code in the "Forum" Form.

ye , about 2 days , 2-3h/day

Guild leader ResQ , his got some items +11 - +13 , so it might be Oriya , dunno for sure
