404 Error I don't know why you would get that, unless you were typing the URL wrong, but if you wanted to put it on a server other than the one with the DB, you'd have to open port 1433 in your firewall

i get an undefined variable error on this line in register.view.php...

Code:
<?php if(count($errors)){ ?>

Are you going to

Are you able to connect remotely to the SQL server using the same credentials as you are for the script, but using SQL Server Management Studio instead?

When I say connect remotely, I mean directly from SSMS, not going through remote desktop.

Also make sure you don't make a public facing server with the default passwords.

got the registration to work but ...when i login = fail. help?

Well, if you're using the same user/pass and it's all correct from what you used on registration, then either you're not logging into your server, or there is an issue in your Login SP.

Check PS_UserData.dbo.Users_Master and see if your account is there, if it is, check your login procedure.

You put php files wherever your web server software serves php files from, usually in a public_html or www folder. It depends how you have it configured and what web server software you are using however.

I've never used PHP with IIS, so you might have to do a little reading on that. You can edit PHP files with any text editor as they are just text files.

You mentioned wampserver and iss(IIS?), does that mean you have two web servers installed on the same machine? You should only need one or the other.
