Both. You can get banned automatically even if nobody reports you if the system somehow thinks you've been cheating. But if you were reported, an EA employee will check your account manually and then decide whether to issue a ban or not.

3 warning = ban I guess

3 times a day in draft

but some times When i'm bored, I use it in the same hour
