Commands /refine or /upitem do exactly the same.
They can have 3 parameters after the command.

First parameter:
/refine 0 <- location of the item in your inventory, 0 means the first slot on the upper left.

Second parameter:

Yes the upgrade can fail just as it could upgrading the item anywhere else, The difference is that you can upgrade any item anywhere :pimp:
