Start the IM before to start S4; to display the IM window during you are in the game server lobby, you must select it by holding down the Alt + Tab keys and use it just as was indicated in the first post by Honeysweet. Hope i helped. :)

I started up the IM before I started S4 and it still did not work.

I also did the way Honey said so by:
4 Start S4 League
5 Start "ItemManager.exe" as Admin

and that did not work either.

I kinda stated my problem incorrectly :/ sorry, so I like click on my profile, and then click the load button, and then go into a room and i still have my tps instead of my profile

Lol you don't need to know any values, just put clothes+weaps you want to,
press "get" in the IM,
remove the clothes+weapons,
put tps\tsmg+15hp
press "set"
enter a room, press restore and play. ^^

My problem:
After i loged and all ... When im going to create my profile it says at the bottom "NOt loged yet Waiting" .. so i cant ._. ..
Helpzor?

EDIT:
Not quiet sure if it has anything to do with this but... My IM is always Open its not on ma "Window bar" its in a corner of ma S4 game, Cant close it .. i can minimize it .. but its stil there in a corner
im not 100%sure if i did all the steps one by one .. but well i did whut the English part said

Sadly, that does not work either.
When I click "Get" nothing happens.

You need to download that file and extract ONLY its xzonelib3.dll to your DLL folder.

And yes, for most people, it works with patch 22.

Ok .. i think i fixed it ..
"Didnt realize that the actually IM was on the "ItemManager" folder.. (the 2nd rar file has one also)
Now i cant get it to work .. i created the profiles and all .. i press Load .. and Nothing ._.
A video doing all the steps one by one would be REALLY helpfull (if someone that made it work is interested)

Item Manager Works 100% in Patch 22.1 ^^

when I start the IM it doesnt find the s4 path...
