Yes, you should always patch. It doesn't matter if it's through autopatch or manually... Some patches overwrite the executable, in which case you'll need to come here and wait for an update (to ease your mind, the last patch that actually overwrote the executable was 6-8 patches ago).
You don't need to do anything to any files, besides patch the client. The virus scanner, 91gamecheck (and its modules), and the other few antibot modules are not loaded in the client itself. It also won't send any information out, so there's no need to block IP ranges.
Basically just patch your client and you're good for future patches... until the executable gets overwritten.
