Hi here I edited the somewhere founded index.php added and deleted some functions and created it for srevolution arcane mysql project, because there are no working mysql reg page. It is very simple, fast and easy to use.

Extract that regpage doc, open index.php and edit these lines:

$dbhost = "localhost"; // let it default (localhost) becouse is need to be working on localhost.
$dbname = "silkroad"; // Your mysql database name like (silkroad, silk, evo and etc..)
$dbuser = "root"; // Your database username default is root
$dbpass = ""; // Your databse password like 123, root. It can be empty

sec i'll convert my one into mysql and upload here.. this one even doesnt have security.. took it and tried sql injection - was pretty successfull dropped all my tables with ease

good luck with that i just dont see the point of mysql regpage if there's mssql version which is about 99,99% safe for injections, and has alot more features and can be easily converted to mysql one with simply changing mssql_connect to mysql_connect..

Full protection against any sql injection code.
If you want to make a mysql version just change the configuration to mysql connect statement
And redo little bit of the query.
